To apply for new service from the LPWDN, please come by the office at 307 Rue Scholastique, Lafayette, LA 70507 to complete an Application for Service. You must also bring your driver's license or other valid pictured ID; and a copy of the lease, act of cash sale, etc., showing proof of occupancy. Fees for service installation, meter service, and a refundable deposit will be due with the application.
For your convenience and to speed up the process for you at the office, an Application for Service can be downloaded and printed. You can fill out all the necessary information on the application form before you get to the office.
